| Cognify Picks Greenway’s Primesuite for Integrated Care |

Greenway Medical Technologies said it has integrated PrimeSUITE, its electronic health record (EHR), practice management and interoperability solution with Cognify’s web-based Participant Tracking System that monitors care plan continuums. Together, the technologies provide an advanced solution to customers, including Total Longterm Care Solutions of Denver, which is one of the nation’s largest elder care providers within the Centers for Medicare and Medicaid Services (CMS) Programs of All-Inclusive Care for the Elderly (PACE) program. PACE regulatory requirements encompass clinical care planning, Medicare and Medicaid covered services, and those deemed necessary by interdisciplinary medical teams such as homecare, transportation, scheduling and prescriptions. Cognify’s Participant Tracking System combined with PrimeSUITE’s functionality allows more than 700 current TLC caregivers the ability to easily coordinate clinical and administrative care plan data to increase efficiencies, lower costs and adhere to PACE program compliance. “Electronic care planning in tandem with clinical staff and federal regulations allows us to clearly document and integrate the many aspects of complex care,” said TLC Vice President for Quality Susan Grayson. “This system has been working reliably and the customer service to make adjustments as new ideas develop has been tremendous.”
